• reconciler reads derived_from.entity — cross-entity provenance reads as ‘cited’, not ‘born’

Bug Fixes

• a pack release could silently overwrite a customer’s own work — add/add is a conflict

• hydration’s silent exit — a skipped step that says nothing reads exactly like one that ran
